Application for student records may be undertaken online through the FEU website at www.feu.edu.ph\online_services\student_records_application.
Procedure:
1. Choose online services from the top menu of the FEU homepage.
2. Click on Student Records application.
3. If it’s a new application, choose “Submit a new application”.
4. Click on NEXT.
5. Select the type of document and the number of copies needed.
6. Click on NEXT.
7. Fill up the Personal Information Sheet.
8. Click on NEXT.
9. Fill up the Academic Information Sheet.
10. Click on NEXT.
11. Provide the contact information and preferred mode of delivery. A representative may be designated in case the user is unable to claim/receive the documents. Two(2) valid IDs of the designated representative are needed when claiming/receiving the documents.
12. If the user opts to have the documents delivered by courier within the Philippines, he must fill in the delivery address in the next screen (with a designated representative, if necessary).
13. Similarly, if the user opts to have the documents delivered outside the Philippines, he/she must supply the address in a separate screen (with a designated representative, if necessary).
14. The next screen will inform the user that prior to processing of application, his/her past dues will be checked for inclusion in the charges to be paid. The user then will choose to click on “I agree” or “I will apply again at a later date”.
15. If the user agrees, he/she is given a tracking code and he/she can check the status of his/her application after 10 working days.
The user may check the status of the application by inputting the tracking code.
The user is shown the amount that needs to be paid and the payment modes are given.After payment, the user can check the status of the application using the same tracking code.
